• Perform social assessment for the identification of social cases among hospitalized or outpatients
• Undertake the necessary steps to prevent chronic patients loss to follow up
• Ensure the social accompaniment of chronic cases
• Coordinate the  health education program in the hospital
• Participate in supervision of health centers social programs
