KADUHA DISTRICT HOSPITAL PATIENTS RIGHTS AND
RESPONSIBILITIES CHARTER
To our esteemed clients,
The management of Kaduha district hospital wishes to inform you that you have right to:
a. Right to dignity and respect
1. Hospital staffs have the responsibility to speak to you politely, in the languages that you understand well.
2. It is your right to be addressed according to your preference (titles such as Dr, Sr, Sir, Madam, …. or names )
b. Right to Care
1. You have the right to receive care when you need it.
2. You have the right to the best quality care available, including preventive, curative, primitive, and rehabilitative care services
3. You have the right to a full continuum of care including referral when needed.
c. Right to Privacy
1. Clinical staffs have the obligation to ensure you are consulted in a private space free from being seen or over heard by others.
2. Only clinical staff involved in your care have access to your health information
3. Health information about your condition can be revealed to only you,and your preferred family members or legally competent. However, if
you are under 18, your parents/legally competent guardians
d. Right to confidentiality
1. All our staffs are aware of their professional and employment obligation to keep confidentiality of your health information.
2. Electronic medical records are protected by passwords and access is reserved for clinical staffs.
3. Access to medical records is only allowed to clinical staffs involved in case management
4. Patients’ charts are kept in appropriate, private place with restricted access.
e. Right to safety and security
1. You have the right to be cared by healthy and competent personnel in a clean and safe environment with the most safe equipment
2. You have the right to be protected from injuries , fire and any type of accident while at the hospital
f. Right to choice
1. You have the right to chose the provider you want to care you
2. Our care providers have the responsibility to allow and guide you making a choice from the possible treatment options
g. Right to information
1. You have the right to know the package of services available at the hospital, when and where as well as what it takes to access it
2. Health care providers have the obligation to inform you of your condition, causes, possible complications and prognosis, treatment options and their effects
3. You have the right to know the names of clinicians treating you
h. Right to complain
1. You have the right to complain to the hospital leadership of what does not please you either verbally, by phone calls, email or paper writing
2. The customer care officer has the responsibility to assist you in the process
i. Right to refuse treatment
1. You have the right to refuse treatment proposed by care providers
2. However if you are under 18, you parents or guardians will help you deciding
j. Right to pain management:
1. Pain is one of the leading causes of consultations at the hospital. We know how bad it feels and believe that pain access to pain treatment
is a human right.
2. You have the right to the best pain medications available.
k. Right to be respected in regard to cultural and spiritual beliefs
1. Our staffs have obligation to respect your cultural and religious beliefs Vis -a -Vis treatments, nutrition and other aspects during care
2. You have the right to be facilitated for worship according to your preferences
3. The hospital allows religious personalities to assist you in regard to your faith when needed (Pastors, Priests, Imams etc)
To enable us to promote your rights, you also have the following responsibilities:
1. Kaduha hospital is not a commercial entity. However, our mission to provide health care to all members of the population cannot be achieved without the financial capacity required to run the facility. We therefore require you to contribute to financial sustainability by
ensuring you pay for all services delivered.
2. You are responsible for taking care of your own health
3. You are responsible to access and use the health system properly and not abuse it
4. You are responsible for respecting the rights of health providers and other patients
5. You are responsible to provide health care providers with relevant and accurate Information about your health condition
6. You are responsible to follow all instructions and adhere to treatments prescribed
